SystmConnect is our secure online portal. It is the quickest and easiest way to contact the surgery for both medical and administrative needs.

Submit a Medical or Admin Request via SystmConnect

1. Submit Your Request 

Please click on the link above. You will be asked a few short questions about your symptoms or your administrative request (such as a sick note or test results). You can submit a request for yourself or on behalf of someone else. 

2. Clinical Review 

Once submitted, your request is sent directly to our triage team. A clinician will review the information provided to determine the most appropriate course of action. 

3. Our Response 

We will contact you with the outcome. This may be: 

  • An invitation for a face-to-face appointment. 
  • A telephone or video consultation. 
  • Advice sent via SMS or email. 
  • A referral to a local Pharmacy or another healthcare service. 

 

Frequently Asked Questions 

What if I don’t have internet access? If you cannot use the online form, please call our reception team as usual. Our staff will ask you the same questions found on the SystmConnect form and enter the details on your behalf. This ensures all patients are triaged fairly, regardless of how they contact us. 

When should I use SystmConnect? You can use it for: 

  • New or ongoing medical symptoms. 
  • Follow-up appointments. 
  • Medication queries. 
  • Fit (Sick) notes. 
  • Test results. 

Is this for emergencies? No. SystmConnect is for non-emergency medical issues and admin. If you are experiencing a life-threatening emergency (such as chest pain, severe bleeding, or signs of a stroke), please dial 999 immediately. 

When will I hear back? We aim to respond to all clinical triage requests via your preferred contact method by the end of the next working day. Urgent requests are prioritised for immediate action.
